Before You Schedule
Thank you for scheduling your pet with Spay Oklahoma for spay/neuter surgery and helping curb the community’s overpopulation of unwanted and euthanized pets.
Appointment requirements

Asterisks indicate required fields. The form cannot be submitted until all required fields are complete.
- vaccinesRabies vaccination
A required rabies vaccination is given to animals over 16 weeks, or at the doctor’s discretion, for $5 unless proof of current rabies vaccination is provided.
- petsMultiple pets
You may schedule multiple pets, but a separate form is required for each pet.

If confirmation is not received within 48 business hours, check your spam folder and email schedule@spayok.net if needed. Requests made Friday through Sunday are approved on Monday.
- event_repeatRescheduling or canceling
Give at least 48 hours’ notice. The live site states a strict one-time reschedule policy.
- paymentsPayment
Payment in full is required before an appointment can be booked.

Completing the form
Name: If unknown, enter “Dog” or “Cat.”
Sex: If unknown, choose Female.
Age and Weight: Estimates are okay.
Breed: Give your best guess rather than “Mutt.” For cats, choose Domestic Short Hair, Medium Hair, or Long Hair when appropriate.
Date: Complete the entire form before selecting a date. You may need to scroll back up to see available dates.
